Our Common Intake Form

The intake process uses Human-Centered Design principles 
and an intuitive interview format. It routes participants to the 
most appropriate programs and services, including job 
matches, based on eligibility criteria. The system also 
leverages Salesforce 360 Truth and API integrations to pull in 
external data, consolidating it into a single customer record. 
Referrals can be made automatically, and subcontractors and 
partners may access and manage participant data as needed.

Job seekers can upload and attach documents during intake, 

such as resumes, birth certificates, or assessments. These 

documents are categorized by type and stored within the 

participant’s record. The system can also initiate additional 

assessments such as ITAs (Individual Training Accounts) 

during or after intake.

Eligibility Screening, Programs 
Job Matching

Launchpad automatically determines program eligibility 
based on intake responses, with validation checks built into 
the backend. These rules are fully configurable and can be 
updated as regulations change. Once intake is completed, case 
managers are presented with a list of programs participants 
are eligible for. 

Launchpad’s matching engine uses recorded Participant Skills and 
compares them to the skills required by job orders added to 
the system. When a match is identified, it is flagged for both 
staff and participants. This matching functionality continues 
post-intake and evolves as more information is entered or job 
orders are added. Staff can approve enrollments, assign participants 
to programs, and configure workflows based on agency preferences. 

Assessments, ITAs, and Referrals

Helping participants reach their goals often involves outside 

resources. The Case Management solution can create

assessment, ITAs, and contract records that can be viewed 

directly from the participants’s profile.

Automated processes can be put into place to make reviews 

and approvals for contracts and funding a seamless and 

hassle free process. Launchpad’s Case Management also 

uses location services to match participants with providers based 

on their proximity.

Follow-Up & Progress Tracking

Launchpad automatically generates follow-up tasks, such as post-exit documentation or training completions. The participant record provides a comprehensive view of enrollments, services, job matches, applications, progress toward goals, and case notes.
